I went to A&amp;E and they ruled out a heart attack. It seems to flare up when I\u2019m stressed. Now I\u2019m waiting for a referral for cardiology. I am 34 and otherwise healthy.\u00a0<\/span>\u2013 <span class=\"mol-style-italic\">Connie Murray, Bicester, Oxfordshire<\/span><\/p>\n<p class=\"mol-para-with-font\"><span class=\"mol-style-bold health-ccox\">Dr Scurr replies:<\/span> It is reassuring that serious causes, including a heart attack, have been ruled out. I imagine that this involved having an electrocardiogram (ECG), which checks the heart\u2019s electrical activity and rhythm, and a blood test for troponins (a protein released from damaged heart muscle).<\/p>\n<p class=\"mol-para-with-font\">But I think more testing is needed as what you\u2019ve described sounds like angina \u2013 heart pain that occurs when there\u2019s insufficient blood supply to the muscle, such as when exercising or stressed. As angina doesn\u2019t directly damage the heart, it won\u2019t show up in these tests.<\/p>\n<p class=\"mol-para-with-font\">Although angina is more common in older age, as fatty plaques build up in the blood vessels, it can occur in younger people \u2013 even at just 34.<\/p>\n<p class=\"mol-para-with-font\">It\u2019s diagnosed with a CT angiogram, which checks how the blood is flowing, or a stress ECG, in which your heart is monitored under \u2018stress\u2019 on a treadmill.<\/p>\n<p class=\"mol-para-with-font\">If there is any restriction of the blood supply to your heart muscle, there are good and effective options such as medication including a statin, or \u2013 if there is serious restriction of blood flow \u2013 the insertion of a stent.<\/p>\n<p class=\"mol-para-with-font\">If angina is ruled out, the pain could be due to oesophageal spasms \u2013 painful contractions of the muscle in the gullet that can be stress-related.<\/p>\n<div class=\"artSplitter mol-img-group\" style=\"\" wp_automatic_readability=\"10\">\n<div class=\"mol-img\">\n<div class=\"image-wrap\"> <img loading=\"lazy\" decoding=\"async\" id=\"i-a31e6c567f8c39ec\" src=\"https:\/\/i.dailymail.co.uk\/1s\/2026\/04\/20\/11\/108017837-15748145-image-m-20_1776681368745.jpg\" height=\"423\" width=\"634\" alt=\"If there is any restriction of the blood supply to your heart muscle, there are good and effective options such as medication including a statin, writes Dr Scurr\" class=\"blkBorder img-share\" style=\"max-width:100%\" loading=\"lazy\" \/> <\/div>\n<\/p><\/div>\n<p class=\"imageCaption\">If there is any restriction of the blood supply to your heart muscle, there are good and effective options such as medication including a statin, writes Dr Scurr<\/p>\n<\/div>\n<p class=\"mol-para-with-font\">Gallstones can also cause chest pain and radiate up to the right shoulder (this is diagnosed with an abdominal ultrasound).<\/p>\n<p class=\"mol-para-with-font\">It may also be pain referred from the spine (although this would be unlikely to cause the tightness in your chest that you describe in your longer letter).<\/p>\n<p class=\"mol-para-with-font\">I suggest you speak to your GP and raise the possibility of the pain being angina \u2013 mention any of these risk factors, if these apply to you: being overweight, smoking, high blood pressure or a family history.<\/p>\n<p class=\"mol-para-with-font\">Whatever the case, you should ask your GP to push your referral to the cardiology department as a matter of urgency.<\/p>\n<p class=\"mol-para-with-font\"><span class=\"mol-style-bold\">Over <\/span><span class=\"mol-style-bold\"><span class=\"mol-style-bold\">the past year<\/span> I have experienced extreme fatigue and tiredness<\/span> \u2013 <span class=\"mol-style-bold\">and this has got worse; I am now tired from the time I wake until the time I go to bed. I have severe sleep apnoea and for the past six months have slept with a face mask. I am 83 and at my wits\u2019 end.\u00a0<\/span><span class=\"mol-style-italic\">\u2013 Name and address supplied<\/span><\/p>\n<p class=\"mol-para-with-font\"><span class=\"mol-style-bold health-ccox\">Dr Scurr replies:<\/span> Obstructive sleep apnoea (OSA) is a common condition in which your breathing is constantly interrupted during sleep.<\/p>\n<p class=\"mol-para-with-font\">It\u2019s often related to obesity, as the excess fat around the neck presses down on airways while you sleep, causing them to collapse. Symptoms include constant daytime fatigue, as well as poor concentration, headaches and low mood. The standard treatment is with a CPAP (continuous positive airway pressure) machine \u2013 the face mask you describe. This delivers constant air to keep the airway open, and I would have expected you to improve within weeks of starting with it.<\/p>\n<p class=\"mol-para-with-font\">You should first check that the CPAP equipment is functioning correctly \u2013 this includes the mask fitting well to prevent air leaking out.
